Andrew Kurmis

Associate Professor Andrew Kurmis is an Australian-trained Orthopaedic Surgeon, based in Adelaide, with subspecialty training expertise in hip and knee replacement surgery.
He is an active researcher working towards improving patient outcomes having published nearly 50 peer reviewed journal articles. He has also presented work extensively within Australia and internationally, often as an invited speaker.
Special Interests include primary & revision hip and knee replacement surgery, Computer-navigated and Robot-assisted surgery, ‘Patient-specific’ implant systems, and General Orthopaedics.

Available Research Projects
Research Project 1 - Recognising the importance of pre-operative iron testing in lower limb arthroplasty patients – THR study.
Project description: Recently highlighted acutely by the global COVID-19 pandemic and fears of allogenic transfusion product shortages, recognition of the importance of effective peri-operative blood preservation techniques has increased. The routine utility of pre- (and peri-) operative iron screening continues to gain clinical interest in many surgical domains, including orthopaedic surgery.
Located within the Northern Adelaide Local Health Network, the Lyell McEwin Hospital is one of the leading tertiary hospital facilities in the country for provision of intra-operative cell salvage and peri-operative blood conservation measures. Supported by a multi-disciplinary clinical team, the hospital continues to explore contemporary methods for safe reducing (or avoiding) the need for blood product transfusion.
The proposed project, performed as a retrospective case-note audit, aims to establish the levels of compliance with guidelines in place for pre-operative iron screening for patients undergoing elective total hip replacement surgery. It is hoped the scientific data obtained from this endeavour will inform future prospective trials utilising blood product conservation measures (including supplemental iron therapy) with the goal of further improving patient outcomes and treatment cost efficiencies.

Research Project 4 - Diagnosing peri-prosthetic joint infection (PJI) in South Australia - application of the current MSIS criteria to retrospective decision-making pathways.
Project description: In 2018 the international Musculoskeletal Infection Society (MSIS) released updated criteria for the diagnosis of prosthetic joint infections (PJIs). Based on a weighted scoring system of clinical, pathological, radiographic and serum markers, the composite MSIS score is proposed to provide clinicians with an evidence-based algorithm for the ‘diagnosis’ of PJI. Well validated in the North American setting, there has been little work to replicate findings from that setting in other parts of the world.
Through means of a retrospective case-note based review, the proposed project aims to audit a series of 100 consecutive ‘infected’ total joint replacement cases managed within the NALHN network and calculating their respective MSIS score. The outcome results of this determination (i.e. infection ’likely’, ‘unlikely’ or ‘indeterminate’), will be contrasted with the actual treatment pathway that the patient was subjected to with a view to assessing if application of the 2018 MSIS criteria would have prospectively change treatment. Where available, the diagnostic utility based on the 2018 MSIS score calculated will also be contrasted with the final pathology specimen –based ‘gold standard’ result.
