Professor Michael Horowitz was appointed to a Personal Chair at the University of Adelaide in 1995 and has been the Director of the Endocrine and Metabolic Unit at the Royal Adelaide Hospital since 1997. His research activities are almost exclusively clinically based and relate primarily to gastrointestinal motor, sensory, and hormonal function, particularly in the context of diabetes mellitus, appetite regulation, critical illness and aging.

Professor Horowitz leads the National Health and Medical Research Council (NHMRC) Centre of Research Excellence (CRE) in Translating Nutritional Science to Good Health (2012-17). He is a co-author of 713 peer-reviewed papers and 41 book chapters. His papers have been cited 48031 times (15419 times since 2015) and his H-index is 114 (Google Scholar). In 2019 Professor Horowitz was ranked 1892 in the world and 42 in Australia.

He has been the recipient of a number of awards, including the Elder Prize for Scholarship at the University of Adelaide in 1995, the Distinguished Research Prize of the Gastroenterological Society of Australia (1999), the Eric Susman Prize of the Royal Australasian College of Physicians (2000), the Kellion Award of the Australian Diabetes Society (2009), a Masters Award for Sustained Achievement in Digestive Sciences from the American Gastroenterological Association (2010), An inaugural Fellow to the Australasian Academy of Health and Medical Sciences (2014), and the Royal Australasian College of Physicians 75th Anniversary Award (2014).

In recognition of his outstanding contributions to the field, Professor Horowitz was awarded an L3 Investigator Grant from the NHMRC (2020-2024) for his project "Gastric emptying, glucagon-like peptide-1 and glycaemic control in diabetes and critical illness".

Research

My major research interests are in the two unrelated areas of gastrointestinal motor function and metabolic bone disease.  In both instances this research is being performed on a collaborative basis and has attracted international recognition.  The high level of scholarship and my pivotal role in both of these research programmes is evidenced by: (i) 212 papers in international, peer-reviewed journals and 22 book chapters.  – The majority of these papers have been published in high impact journals such as Gastroenterology, Gut, the American Journal of Physiology, Diabetologia, the American Journal of Clinical Nutrition and the British Medical Journal and they were cited in the scientific literature 311 times in 1993 and 380 times in 1994;  (ii) considerable success in attracting funding from competitive sources.  I have been the recipient of a total of approximately $1,978,000 from external peer-reviewed granting bodies since 1994 including 11 project grants, each of three years duration, provided by the National Health and Medical Research Council of Australia and grants from the Dutch government and the US Department of Health and Human Services;  (iii) the ongoing involvement of established overseas researchers of the highest calibre;  (iv) multiple invitations to participate in both national and international meetings, and;  (v) the ability to attract both Australian and overseas research fellows (the latter usually provide their own funding).

Originally funded by the National Health and Medical Research Council (NHMRC), the CRE seeks to encourage and promote clinical research in the area of nutritional physiology. The Centre aims to formalise existing collaborations and facilitate the development of others, which span the spectrum from basic science, through human physiology and clinical medicine, to population health.

Our investigators, with backgrounds in endocrinology, gastroenterology, nutritional science, nuclear medicine, psychology, epidemiology and nursing, bring unique technical skills, unparalleled in this country, and a sustained record of productivity in clinical nutritional research, attested to by a high international profile, substantial impact on clinical practice, and the capacity to communicate a healthy nutrition philosophy to the public.

Research Project 1

Title: The Augmented versus Routine approach to Giving Energy Trial (TARGET).

Project description: TARGET is a 4,000 patient, multicentre, double-blinded, randomised, controlled, parallel-group, phase III clinical trial. The primary aim of the study is to determine if augmentation of calorie delivery using energy dense enteral nutrition in mechanically ventilated patients improves 90 day survival when compared to routine care. A secondary aim is to determine if augmentation of calorie delivery using energy dense enteral nutrition in mechanically ventilated patients improved functional outcomes at 180 days when compared to routine care. Eligible patients will be randomised 1:1 to receive TARGET protocol enteral nutrition (EN) formulation of either 1 kcal/ml or 1.5 kcal/ml. Protein content in both TARGET protocol EN formulations will be similar. TARGET protocol EN will be administered for up to 28 days during the ICU stay.

Research Project 2

Title: Gastric emptying, glucagon-like peptide-1 and glycaemic control in diabetes and critical illness

Project description: Gastric emptying and the ‘incretin’ hormone, glucagon-like-peptide-1 (GLP-1), are central to blood glucose regulation and management of diabetes. The rate of emptying may be influenced by ethnicity. Delayed emptying (gastroparesis) is frequent in longstanding type 1 (T1D) and type 2 diabetes (T2D) but, even when normal, gastric emptying is a major determinant of the glycaemic response to a meal. In insulin-treated patients gastroparesis may increase the risk of hypoglycaemia. Acute hyperglycaemia slows, while hypoglycaemia accelerates, emptying. The latter may represent an important counterregulatory mechanism, particularly when awareness of hypoglycaemia is impaired. GLP-1 receptor agonists (GLP-1RAs) given subcutaneously, are used widely in T2D. ‘Short-acting’ GLP-1RAs slow gastric emptying markedly, which is the major mechanism for lowering postprandial glucose. While effective, the response to GLP-1RAs is highly variable, and comparisons between GLP-1RAs are compromised by ‘open-label’ studies. Intravenous administration may be more effective than subcutaneous for glucose-lowering by GLP-1; the latter route may contribute to an inadequate response to GLP-1RAs. Stress hyperglycaemia is frequent in the critically ill and managed with insulin, with a high risk of hypoglycaemia. The proposed studies will evaluate: (i) gastric emptying and GLP-1 secretion in indigenous Australians without diabetes; (ii) gastric emptying and the response to hypoglycaemia in T1D with impaired awareness of hypoglycaemia; (iii) the individualised use of GLP-1RAs in T2D based on glycated haemoglobin and gastric emptying; (iv) the comparative effects of intravenous and subcutaneous administration of GLP-1 in T2D; (v) the mechanisms underlying suboptimal response to GLP-1RAs in T2D; and (vii) the effects of GLP-1 in critically illness with ‘stress’ hyperglycaemia.

Teaching activities

During all stages of my postgraduate career I have made major contributions and enjoyed both undergraduate and postgraduate teaching.  My involvement in postgraduate teaching on both a national and international basis is increasingly sought and I have consistently received excellent ratings for my teaching from both medical students and physician trainees.  Major initiatives include:

  1. International training in gastrointestinal motility
  2. Establishment of research project in the medical curriculum
  3. Contributions to professional literature and editorial boards
  4. Royal Adelaide Hospital Research Forum

In addition to those stated my current activities in relation to teaching include:

(i)         Postgraduate

(a)  Tutorials in general medicine and endocrinology to trainees undertaking the Part I examination for the Fellowship of the Royal Australasian College of Physicians (about 2 hours/week);  (b) invited lectures at local, national and international meetings (including lectures to interns and regular presentations at medical grand rounds at the Royal Adelaide Hospital) (about 3 hours each week);  (c) supervision of higher degree students; and (d)  supervision of postdoctoral research fellows.

 

(ii)         Undergraduate

Tutorials (mainly bed-side teaching) and lectures in general medicine and endocrinology (metabolic bone disease, pituitary function, diabetes, gastrointestinal disorders) to 3rd, 4th, 5th and 6th year medical students (about 8 hours/week).  This includes weekly tutorials (about 3 hours) to 4th year medical students during their attachment to the Professorial Medical Unit.  The Departmental student assessments of these tutorials has been extremely favourable;  (b)  examination of 4th, 5th and 6th year medical students.   I have had a major involvement in the 4th and 6th year medicine examinations and organised the clinical component of the 4th year examination at the Royal Adelaide Hospital on a number of occasions.
